After giving them this specific item, reload the area to find them turned into an enemy. Slay them and then loot their corpse for the 5 precious scales.

Fountainhead Palace (23 Scales)

Easily the location in Sekiro where one will find the most Treasure Carp Scales, Fountainhead Palace has a staggering 23 of them scattered about its flooded expanse.
